Healthcare Leadership and Patient Care Coordinator

Overview:
At UnityPoint Health, we're seeking an Advanced Practice Nurse/Stroke Coordinator/Acute Adult to join our team. This role will play a vital part in supporting the Director and Managers within the division, as well as across patient care services.
Responsibilities:
- Demonstrate clinical leadership as a role model for staff nurses.
- Build partnerships that support and promote care coordination and care management strategies.
- Consult with nursing management regarding issues or barriers in delivery of care, staff achievement of competency, system concerns, provider concerns, or specific patient care problems.
- Participate in divisional and hospital shared governance infrastructure.
- Coordinate and lead hospital-wide patient initiatives, including delirium, pain, sedation, and diabetes.
Clinical Excellence:
- Facilitate implementation of evidence-based practice or nursing research studies and translate research findings into best nursing practice for care.
- Partner with providers and patient care team members to achieve progress towards outcomes.
- Maintain current knowledge of trends and advances in nursing practice.
- Use advanced concepts in nursing processes in the clinical setting.
- Incorporate the St. Luke's Hospital's Comfort/C.A.R.E. Professional Practice Model into daily nursing practice.
Education:
- Serve as a resource to the nursing staff and other healthcare associates based on professional standards of care.
- Assist with formulation of evidence-based nursing policy and procedure.
- Provide support and resources to the division for on-boarding new associates.
- Support competency development and implementation within the division for nursing staff, including Patient Care Techs.
- Serve as a resource for other members of the healthcare team, including providers, who are involved with performance improvement tools.
Qualifications:
- Completion of an accredited nursing program.
- Master's degree in nursing or healthcare-related field.
- Three years of clinical experience.
- Current RN licensure in the State of Iowa or compact state.
- Current BLS certification.
- Writes, reads, comprehends, and speaks fluent English.
- Multicultural sensitivity.
- Microsoft Office - basic computer skills.
- Customer/patient focused.
- Use of usual and customary equipment used to perform essential functions of the position.
Estimated Salary: $120,000 - $150,000 per year, depending on experience.
