Job SummaryThe Licensed Clinical Social Worker is a key member of our healthcare team, responsible for providing initial evaluations and individual and group therapy to patients. This role requires a strong understanding of social determinants of health and the ability to provide care coordination and management to patients with complex needs.
Key Responsibilities:- Conduct initial evaluations and assessments of patients' mental status as directed by their primary care physician.
- Select and implement appropriate therapeutic interventions based on clinical knowledge and educational background.
- Provide individual, family, and group therapies tailored to patients' developmental and behavioral health needs.
- Collaborate with physicians and other healthcare team members to ensure seamless care coordination.
- Ensure accurate and timely documentation of patient progress in medical records.
- Stay up-to-date with community-based and government services to support patients in accessing resources and addressing social determinants of health challenges.
- Master's degree in social work.
- Current licensure as a Licensed Clinical Social Worker with at least three years of behavioral healthcare experience.
- Experience in a primary healthcare setting is preferred.
- Bilingual in Spanish is a plus.
