Job Summary

As a Registered Nurse with Prisma Health, you will play a vital role in delivering exceptional patient care in our acute inpatient setting. Your clinical expertise and compassion will inspire health and serve with purpose. You will foster patient-centered care, utilizing evidence-based practices to assess, plan, implement, and evaluate patient outcomes. Your knowledge of safety protocols, cost containment, and quality improvement will ensure the highest standards of care. You will identify patient, unit, and organizational priorities and incorporate them into the nursing process.

You will adhere to all compliance and policy/procedures of the organization, including licensure renewal, assigned training, employee health screenings, time and attendance policy, dress code policy, patient confidentiality, infection control, and medication administration. You will demonstrate knowledge of regulatory agency requirements (TJC, SC DPH, CMS) and provide care within the scope of the South Carolina Nurse Practice Act.

The acute inpatient setting may require staff to be scheduled for a weekend, holiday, and on-call work.

Accountabilities

Assessment:

Collect data and complete nursing assessments using appropriate area and/or age-specific standards/policies and tools. Complete necessary forms. Assess and recognize subtle or actual changes in patients' status and patient response to intervention. Identify nursing priorities for patient plan of care based on nursing assessment and other data as appropriate. Validate these priorities with healthcare team members and document appropriately.

Planning/Outcomes:

Develop an individual plan of care based on the age and assessed needs of the patient. Develop the plan in collaboration with the patient and/or family, the provider, and other members of the interdisciplinary healthcare team.

Update the plan based on changes in the patient's condition and communicate changes in the plan to other members of the team.

Identify realistic and measurable goals and outcomes to be attained prior to discharge. Validate these with the patient and/or family.

Demonstrate respect for the patient's/family cultural, spiritual, and belief system in planning for care.

Include planning for discharge to assure continuity of care and a smooth transition post-discharge from one level of care to another, including appropriate referrals.

Implementation:

Provide direct care to assigned patients consistent with the established nursing plan.

Perform specific tasks based on organizational, departmental, and/or unit policies and procedures within the RN scope of practice.

Delegate nursing tasks as appropriate to members of the healthcare team, developing written assignments (if appropriate) and ensure completion of assigned tasks.

Monitor quality of care delivered on a continual basis; ensure appropriate surveillance of assigned patients.

Provide care based on patient and unit priorities, coordinating and organizing tasks as appropriate, utilizing resources internal and external to the unit. Incorporate cost containment principles into nursing care. Utilize appropriate technology in daily practice.

Assume responsibility for patient safety by maintaining a safe environment, including awareness of safety procedures, protocols, and adherence to patient care standards.

Utilize performance indicators to collect data, monitor quality, and effectiveness while providing nursing care.

Complete assigned hospital/unit performance improvement monitoring (audits).

Evaluation:

Based on assessment/reassessment of patient response to interventions, evaluate the plan of care for effectiveness, document progress toward expected outcomes, and revise the plan of care as needed. Report changes in patient condition and refer unresolved problems to appropriate healthcare team members. Discuss outcomes with peers (via shift-to-shift report) and with other healthcare team members.

Evaluate opportunities for quality improvement; based upon evaluation, incorporate performance improvement recommendations into the plan of care as appropriate.

Patient Family Education/Teaching:

Identify/assess patient/family learning needs and teach accordingly by:

-using language and terminology that the patient/family is familiar with

-providing examples to clarify ideas

-implementing and evaluating teaching plan (encouraging patient/family involvement/participation)

-Computer resources and Micromedex utilization

Communicate the patient/family's need for education/teaching to others in the healthcare continuum.

Professional Accountability:

Evaluate own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations, and current specific knowledge. Take personal accountability for maintaining competency through education and professional activities. Maintain professional licensure and complete required mandatory education in a timely manner. Reflect on identified areas of strength that offer opportunity for improvement.

Ensure that written documentation and oral communication focus on pertinent information to support meeting patient/family care needs.

Serve as the patient's advocate and coordinator of care, working collaboratively with providers and other members of the healthcare team.

Minimum Hiring Requirements

Associate's Degree in Nursing. BSN preferred.

One year experience as a registered nurse.

Required Licensure/Registrations/Certifications

Licensed to practice as a Registered Nurse in South Carolina

In Lieu Of The Above Minimum Requirements

In lieu of 1 year of registered nurse experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency from another organization.

In lieu of 1 year of registered nurse experience, will accept 1 year of licensed practical nurse (LPN) experience.
