This position is paid on a per-visit basis. The compensation reflected on this posting is an estimate of annual compensation.
Job SummaryWe are committed to providing our patients with the highest level of care and support. As a RN Case Manager, you will be responsible for assessing patient needs, developing care plans, and coordinating with healthcare providers to ensure seamless care delivery.
Responsibilities- Assess patient needs and develop individualized care plans
- Coordinate with healthcare providers to ensure timely and effective care delivery
- Provide education and support to patients and their families
- Monitor patient progress and adjust care plans as needed
To be successful in this role, you will need:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
- Licensed to practice as a registered nurse in the state of agency operation
- Must possess and maintain valid CPR certification while employed in a clinical role
- Ability to travel to all business locations
