Job Title: Supervising Supports Planner

Service Coordination, Inc. (SCI) is seeking a highly skilled and experienced Supervising Supports Planner to join our team. As a Supervising Supports Planner, you will play a critical role in ensuring the highest quality services are provided to individuals with disabilities, behavioral challenges, medically complex needs, and other populations.

Key Responsibilities:
  • Manage the performance of Support Planners to ensure work meets Quality Person-Centered standards.
  • Ensure Supports Planners are using person-centered practices and principles in all aspects of their role.
  • Monitor Key Performance Indicators (KPIs) and complete monthly quality checks and audits for oversight and performance evaluations.
  • Conduct interviews for vacant Supports Planner positions and partner with People & Culture on all recruitment initiatives.
  • Conduct 6-month and annual performance evaluations for Team Members and utilize situational leadership to develop Team Members' knowledge and skills.
  • Take disciplinary actions consistent with SCI policies and procedures to address performance concerns.
  • May provide support for a small number of people supported on occasion.
  • Coordinate and oversee new Maryland Department of Health (MDH) referrals and Supports Planner Transfers.
  • Responsible for updating Compliance Trackers and ongoing monitoring using the Microsoft 365 Suite and other internal and external databases.
  • Prepare and conduct regular supervisions and check-ins for Team Members.
  • Participate in organizational and program meetings, work groups, and committees to stay informed and contribute to policies, procedures, and practices.
  • Communicate SCI mission, vision, and values to agencies, the people we support, and their families and service providers.
  • Speak to community groups to explain and communicate SCI purposes, programs, and policies.
  • Form cooperative relationships with external stakeholders.
  • Participate on and organize external task forces and work groups.
  • Support SP training by reinforcing topics covered in New Team Member Training, reviewing SCI policies and procedures, coordinating shadowing opportunities, observing, identifying, and delivering training based on strengths and areas in need of development.
Requirements:
  • Bachelor's degree in a human services field required; Master's degree in a human service field preferred.
  • At least 3 years' case management manager experience; OR 1 year of case management experience with 3 years of Leadership Experience.
  • Direct experience and ability in working with people who have disabilities, the elderly, those with behavioral challenges, those with medically complex needs, and other populations, and ability to advocate on their behalf.

Physical Demands:

Generally, presents standard office environment. Must be able to remain in a stationary position >95% of the time. Continually operates a computer, keyboard, and mouse, and other office productivity machinery, such as a calculator, copy machine, printer, and mobile Smartphone device. Constantly positions self to comfortably maintain computer equipment. May occasionally be required to move about in an office setting or position office equipment weighing between 10-25 pounds. Vision abilities required by this job include close vision. Ability to operate a motor vehicle. The person in this position frequently communicates with other people. Must be able to exchange accurate information in these situations.
