Position Overview:
The RN Case Manager plays a vital role in coordinating patient care and conducting comprehensive nursing assessments to identify the needs of patients and their families. This position involves collaboration with an interdisciplinary team to develop and implement effective care plans.

Key Responsibilities:
1. Conduct in-home assessments for established patients, ensuring thorough evaluations at each visit.
2. Perform urgent assessments to prevent hospital admissions, prioritizing patient care at home.
3. Continuously evaluate the effects of chronic illnesses on patients' overall well-being, addressing physical, emotional, and social needs.
4. Identify caregiver stress and provide resources to support their well-being.

6. Conduct advanced care discussions with patients and families, documenting their preferences accurately.
7. Educate patients and families about disease management, symptom control, and end-of-life considerations.
8. Initiate preventive nursing procedures when necessary.
9. Participate actively in interdisciplinary team meetings, contributing to patient care strategies.
10. Maintain effective communication with healthcare providers regarding patient needs and care outcomes.
11. Document all patient interactions and progress in the electronic health record.
12. Manage a caseload effectively, ensuring timely follow-ups and documentation.

Qualifications:
- Current Registered Nurse license in the state of employment.
- BLS certification or ability to obtain within a specified timeframe.
- Minimum of 3 years of experience in a clinical setting.
- Strong nursing skills and proficiency in technology.
- Reliable transportation for travel to patient locations.

Preferred Qualifications:
- Certification in Hospice and Palliative Care Nursing.
- Experience in home healthcare and advanced care planning.
- Bilingual proficiency is a plus.

Physical and Mental Requirements:
- Ability to lift and move patients as needed.
- Capability to work in various environments and maintain focus.
