Job Summary

We are seeking a highly motivated and compassionate Service Coordinator to join our team at The Salvation Army USA Western Territory. As a Service Coordinator, you will play a vital role in identifying the human service needs of our residents and developing service plans to support their independence and well-being.

Key Responsibilities
  • Identify Resident Needs: Conduct regular assessments to identify the needs of our residents, including issues related to aging in place, disability, mental health, and abuse.
  • Develop Service Plans: Create personalized service plans to address the identified needs of our residents, ensuring they have access to necessary resources and support.
  • Connect Residents with Services: Collaborate with local service agencies and resources to provide on-site or referral-based services to our residents, promoting their independence and well-being.
  • Educate and Support Staff: Educate our staff on service provision and issues related to aging in place, ensuring they are equipped to support our residents effectively.
  • Assist with Resident Independence: Work closely with our Manager to address issues related to resident independence, ensuring our residents receive the support they need to thrive.
Qualifications
  • Experience: 2 years of experience in a related field, preferably in gerontology, social work, or psychology.
  • Knowledge: Knowledge of Fair Housing laws and regulations, HUD program requirements, and local service agencies and resources for the elderly.
  • Education: Bachelor's Degree in Gerontology, Social Work, Psychology, or a related field.
