Job Summary

We are seeking a skilled Physician to join our team as a Neighborhood Center Physician. This role will involve managing patients in a multidisciplinary value-based care model, collaborating with a care team, and providing 5-Star care to our patients.

Responsibilities
  • Care for Suvida Healthcare's patients and focus on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
  • Manage patients in a multidisciplinary value-based care model.
  • Serve as the leader for their respective care team within the Neighborhood Center.
  • Collaborate effectively with Suvida Healthcare's multidisciplinary care team.
  • Focus on providing 5-Star care and strive to provide a high level of customer service.
  • Keep a medical history and medical records.
  • Refer patients to specialists as needed.
  • Attend daily morning huddles with the care team to discuss patients to be seen that day and work to close any potential care gaps.
  • Attend weekly huddles with the multidisciplinary care team to review and manage high-risk patients within the care team.
  • Begins to influence department's strategy.
  • Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction.
  • Exercises considerable latitude in determining objectives and approaches to assignments.
  • Oversees Advanced Practice Providers as needed within their Neighborhood Center.
  • Reports to the Neighborhood Center Medical Director.
Requirements
  • 3+ years minimum experience, preferably in geriatrics.
  • Active, unrestricted medical license in Texas (for Tx. Centers) or Arizona (for Az. Centers).
  • Current on all required immunizations.
  • A working understanding and support of a managed care, full risk, value-based care model preferred.
  • Strong leadership, customer service, and spoken/written communication skills.
  • Must have the ability to demonstrate self-confidence and positive attitude toward self and others.
  • Excellent team player and communication skills required.
  • Proficiency in Microsoft office platforms including word, excel, and power point.
  • Bilingual/Bicultural (English and Spanish) Preferred.
Education, Experience, Licensure, or Certification Requirements
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.).
  • Board certification or eligibility in Family or Internal Medicine.
  • Current DEA license.
