**Key Responsibilities:**

  • Collaborate with healthcare providers, patients, and hospital staff to navigate the healthcare system and support care management activities.
  • Identify and connect patients with available resources to prevent readmissions and improve health outcomes.
  • Perform complex administrative duties, including data analysis and reporting.
  • Develop and maintain strong relationships with healthcare providers, patients, and hospital staff to ensure seamless care coordination.

**Requirements:**

  • LPN or RN License in CT required.
  • Minimum 3 years of nursing experience in an acute care, post-acute, or home health capacity.
  • Knowledge of regulatory guidelines and ability to apply them in practice.
  • Excellent interpersonal and communication skills.
  • Ability to work collaboratively as a team member and exercise sound judgment in intervening with physicians and other clinicians.

**Preferred Qualifications:**

  • Proficiency in Microsoft Excel and ability to learn new computer software programs.
  • Experience with care coordination and patient advocacy.

**Physical Demands/Working Conditions:**

  • Ability to travel frequently to hospitals, skilled facilities, patient homes, and other sites where patients receive care.
