  • Develop and Implement Care Plans: Develop and implement individualized care plans that address the physical, emotional, and social needs of patients, with the goal of achieving optimal health outcomes.
  • Collaborate with Healthcare Team: Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide Education and Support: Educate patients and their families on disease management, medication adherence, and healthy lifestyle habits, promoting self-care and empowerment.
  • Document Patient Progress: Accurately and thoroughly document patient progress, including assessments, interventions, and outcomes, to ensure continuity of care.
Requirements
  • Current RN License: Possess a current Registered Nurse (RN) license for the state in which you practice.
  • Health Certificate: Hold a current Health Certificate, as required by facility contract or state regulation.
  • PPD or Chest X-Ray: Have a current PPD or Chest X-Ray, as required by facility contract or state regulation.
  • BLS Certification: Possess a current BLS (Basic Life Support) certification.
  • Experience: One year of prior RN experience is preferred, but not required.
