About the Position

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to address them.

You will work collaboratively with our team of clinicians and healthcare professionals to deliver high-quality care and improve patient outcomes. This is a flexible, PRN/Part Time opportunity that allows you to work on your own schedule and choose the cases that interest you most.

Responsibilities
  • Conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans
  • Work collaboratively with our team of clinicians and healthcare professionals to deliver high-quality care and improve patient outcomes
  • Develop and implement care plans that address patients' unique needs and health risks
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and new developments in care management
Qualifications
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
Travel Requirements

This position requires the ability to travel to patients' homes and communities. You must have a valid state driver's license and be able to drive a car. Proof of adequate automobile insurance coverage is also required.
