About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Evergreen Health Services. As a key member of our multidisciplinary team, you will provide primary healthcare services to patients, working collaboratively with other healthcare professionals to deliver exceptional patient-centered care.

Key Responsibilities
  • Assess and evaluate patients' physical and psychosocial status through interviews, health histories, physical examinations, and diagnostic tests.
  • Develop and implement therapeutic plans in accordance with medical protocols, ensuring the ongoing healthcare needs of patients are met.
  • Record findings of physical and developmental assessments in a timely and accurate manner.
  • Identify abnormalities requiring further study and proceed with follow-up care.
  • Initiate first-line medical care plans within defined protocols and discontinue therapeutic regimens as necessary.
  • Develop primary care health regimens for patients using a multidisciplinary team approach.
  • Make appropriate referrals to other healthcare providers and consult with or refer patients to physicians as needed.
  • Initiate and provide emergency care services as required, following established protocols.
  • Coordinate disease prevention and health maintenance programs using available resources.
  • Provide psychological support and counseling to patients regarding health maintenance, stabilization of illness, and prevention of future disability.
  • Perform technical procedures as delineated by privileges granted.
  • Evaluate the plan of care for effectiveness and make necessary adjustments or corrections.
  • Assume primary healthcare responsibilities for patients and monitor the coordination and implementation of care.
  • Participate in planning with other staff in Primary & Specialty Care to develop and monitor required services for patients.
  • Assist in providing learning experiences for clinical staff and other allied health professionals.
  • Actively strive to improve patient service delivery through participation in QI/QA activities.
Requirements
  • Current license to practice as a registered nurse in New York State.
  • Graduate of an accredited college or university with a Master's degree in nursing.
  • Successful completion of a Family/Adult Nurse Practitioner training program as part of a degree program or as approved by the New York State Education Department.
  • Certification by the American Nurses Association or other approved certifying agency as a Family Adult Nurse Practitioner.
  • Certification as a Nurse Practitioner by the State of New York or eligibility for certification.
  • Current BLS and CPR certifications.
  • Experience utilizing EMR software (Medent, preferred).
  • Sensitivity toward cultural diversity, HIV/AIDS, and lifestyle and addiction issues.
