Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with our interdisciplinary healthcare team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Maintain current, active BLS certification through the American Heart Association.
  • IV Certification preferred.
  • Two years recent nursing experience in acute care, long-term care or home health preferred.
  • Valid driver's license.
  • Reliable transportation with proof of insurance.
Working Conditions

This is a full-time position, with a flexible schedule that may include evenings, weekends, and holidays. You will be required to travel within our designated service areas, and may be subject to frequent interruptions and changes in priority throughout the assigned shift.
