Job SummaryWe are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. As a Hospice Nurse - After Hours, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
- Collaborate with the care team to coordinate and revise patient care plans, ensuring continuity and effective communication.
- Regularly communicate patient progress to the clinical manager, physician, and care team to ensure seamless care delivery.
- Develop, prepare, and maintain accurate and timely patient care progress records, adhering to care center policies.
- Record pain and symptom management changes and outcomes as necessary.
- Educate families on patient condition, needs, and care to ensure informed decision-making.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- One+ year of nursing experience required.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
