End of Life Care Specialist

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at West Virginia Hospice in Buckhannon, WV. As a key member of our hospice care team, you will provide end-of-life care expertise and develop trusting relationships with patients and their families.
This is a full-time position involving home visits for hospice patients. You can expect:

The Registered Nurse Case Manager plays a critical role in delivering high-quality patient care in our hospice setting. This includes assuming full nursing responsibility for the delivery of individualized Plans of Care, continuously evaluating personal and professional performance, and making necessary changes to increase productivity and quality of care delivered.
Key responsibilities include:
- developing and implementing treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being
- striving for independence and sound critical decision-making regarding the delivery of patient care in a cost-effective manner without compromising quality
- adhering to and supporting the policies/procedures/goals/objectives of hospice to assure quality patient care
Requirements for this role include:
- one year of clinical experience as a Registered Nurse
In addition to clinical experience, candidates must hold current RN licensure in the state of practice and possess current CPR Certification. A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are also required.
