Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. This dynamic role requires a professional nurse to work closely with patients, families, and healthcare teams to provide personalized care and support.
Key Responsibilities:
- In this role, you will assess patient needs, develop care plans, and collaborate with interdisciplinary teams to ensure seamless transitions across the care continuum.
- You will also provide education and support to patients and their families, promoting autonomy, dignity, and holistic care.
- Fostering a culture of inquiry, you will explore, integrate, and disseminate research and evidence-based practice to drive improved patient outcomes.
- Your expertise will be utilized to lead and coordinate teams, delegate tasks, and facilitate effective communication among care providers.
Requirements:
- Graduate from an accredited nursing program is required.
- Washington Registered Nurse License upon hire is mandatory.
- National Provider BLS - American Heart Association certification is necessary upon hire.
- One (1) year of Nursing experience is preferred.
