Responsibilities- Conduct home visits to assess and treat clients with physical therapy needs
- Develop and implement individualized care plans to improve clients' mobility, strength, and overall health
- Collaborate with other healthcare professionals to ensure comprehensive care
- Provide education and training to clients and their families on proper care and rehabilitation techniques
- Document client progress and maintain accurate records
- Current VA Physical Therapist license
- Minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
