Key Responsibilities:+ Assess physician medication orders for appropriateness regarding drug selection, dosage, potential interactions, and administration routes. Provide recommendations for therapy adjustments to physicians and facility staff as necessary.
+ Maintain a comprehensive patient database, including current and historical medical information, relevant clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).
+ Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, assessed, and discussed with relevant healthcare team members.
+ Collaborate with the healthcare team to deliver clinical and technical support as required.
+ Document all communications with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical record.
+ Implement policies and procedures for the safe procurement, storage, distribution, and disposal of medications and biologicals.
+ Obtain and maintain all required documentation in compliance with federal and state regulations and organizational policies.
+ Notify the Pharmacy Manager of any medication-related errors.
+ Assist facility staff with procedures related to the safe and effective management of medications.
+ Perform additional tasks as assigned.
Qualifications:
+ Bachelor’s Degree in Pharmacy; Pharm D preferred.
+ Minimum of one year of experience in long-term care settings.
+ Licenses/Certifications as required by regulatory standards and business needs.
+ Detail-oriented, goal-driven, team-oriented, with strong oral communication skills.
