Agricultural Outreach Coordinator

CareSouth Carolina seeks a highly motivated and detail-oriented Agricultural Outreach Specialist to join our team. The successful candidate will be responsible for providing direct outreach, case management, and health education services to ensure access to quality, enabling services, culturally competent primary and preventive health care services to H2A/H2B, migrant, seasonal farmworkers and their families within the CareSouth Carolina service area.
Key Responsibilities- Conduct regular visits to farms, labor camps/campsites, and other direct contacts with individuals in the agricultural farm work community to identify health problems and concerns.
- Market health care services to eligible clients, growers/crew leaders, and other farmworker advocates/service providers.
- Identify individual farmworkers and their families in need of health care services and provide referrals to CareSouth Carolina or another appropriate health care provider.
- Determine client eligibility for services and provide a Farmworker Health Information Card to eligible clients.
- Maintain effective communication and service coordination, including Spanish/English interpretation/translation services as needed.
- Refer clients to appropriate health service providers and provide diligent follow-up and tracking of each service to ensure that appointments are kept and adherence to clinical directives.
- Assist members of the agricultural community with addressing social barriers to care and ensure the farmworker and their families receive needed services by navigating the service systems and connecting them to the right resources.
- Coordinate, assist with planning, and participate in agricultural farmworker health clinic and screening events, as well as other community events and programs.
- Collect outreach data, provide activity reports, health screening data, referrals, conduct farmworker needs assessments, survey results, and other information required and related to agricultural farmworker activities.
- Complete Relias trainings and sign off on PowerDMS documents in a timely fashion.
- Perform other duties as requested.
- A valid driver's license is required.
- The ability to travel is required.
- One to three years of experience in the field is required.
- Proficiency in speaking and writing in both English and Spanish is required.
- Strong interpersonal, communication skills, and the ability to work effectively with a range of constituencies in a diverse community are required.
- Must possess strong customer service skills.
- The ability to work after hours and on weekends is required.
