We are seeking a skilled Clinical Social Worker to join our Medical Staff Services team at UCHealth. Key Responsibilities- Assessment and Intervention: Conduct thorough assessments of patients and their families to identify social, emotional, economic, and environmental challenges. Develop and implement effective intervention plans to address these challenges and promote overall well-being.
- Case Management: Participate in case reviews to evaluate care management and progress toward goals. Collaborate with healthcare team members to promote, monitor, and evaluate compliance with patient treatment plans.
- Discharge Planning: Assist with discharge planning and processes, identifying appropriate resources and providing referrals as needed.
- Supervision and Training: Supervise and train new staff, lower-level social workers, students, or interns, providing guidance and support to ensure successful onboarding and development.
- Education: Master's degree in Social Work
- Licensure: State licensure as a Licensed Clinical Social Worker (LCSW)
