About the Role

We are seeking a compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic hospice team at Sangre de Cristo Community Care. As a Hospice LCSW, you will play a pivotal role in providing comprehensive psychosocial assessments, counseling, and support services to patients and families coping with terminal illness.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patient and family needs, strengths, and resources.
  • Provide empathetic counseling and emotional support to patients and families facing end-of-life challenges.
  • Assist with advance care planning, including discussions around goals of care, end-of-life wishes, and decision-making.
  • Facilitate family meetings and support groups to promote communication and coping strategies.
  • Advocate for patients' rights and ensure their preferences are respected throughout the hospice journey.
  • Collaborate with community resources to address financial, legal, housing, and other social determinants of health.
  • Participate in interdisciplinary team meetings to coordinate care and optimize patient outcomes.
Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current LCSW licensure in the state of Colorado.
  • Minimum of 1 year of experience in hospice, palliative care, or related field preferred.
  • Strong clinical assessment, counseling, and communication skills.
  • Ability to work effectively within an interdisciplinary team.
  • Compassion, empathy, and a deep commitment to supporting patients and families during end-of-life transitions.
