V-Tech Solutions, Inc. is seeking a highly skilled and detail-oriented Licensed Case Manager to join our team. As a key member of our care management team, you will be responsible for providing comprehensive coordination services to our clients.
Key Responsibilities:
- Care Coordination: Develop, implement, and update individualized comprehensive care coordination plans for assigned clients in collaboration with primary care physicians and other multidisciplinary team members.
- Client Assessment: Assess clients on enrollment and at intervals determined by stratification level to identify needs, barriers, and gaps in care.
- Care Management: Manage a caseload as outlined by CASSIP and Care Management Leadership, maintaining compliance with contractual and care management requirements.
- Communication: Routinely communicate with clients on their assigned caseload to assess their needs, develop, monitor, and evaluate treatment plan progress.
- Referral and Coordination: Assist clients in planning for transitions of care, including transitioning from early intervention to DCPS, from pediatric to adult providers, and entering or exiting institutional care.
- Documentation: Accurately and timely document client findings and interactions according to organizational policy.
Requirements:
- Licensure: Must be licensed in DC as a Social Worker (LICSW, LGSW, or RN).
- Education: Master's Degree Required.
- Experience: 3 years of experience in healthcare or related experience in managed care and/or care management.
- Skills: Effective verbal and written communication skills, bilingual (Spanish speaking) preferred, familiarity with medical terminology, and computer systems and applications, particularly in Microsoft Office suite.
