Job Summary

We are seeking an experienced RN Clinical Liaison to join our dynamic team. As a key member of our hospice care team, you will play a vital role in educating healthcare professionals about our services, providing end-of-life solutions with unwavering commitment to clinical excellence, compassion, and dignity. Your primary responsibility will be to facilitate effective intake and referral processes, ensuring seamless transitions for patients and their families.

About You

To excel in this role, you should possess:

  • ">
  • A valid RN license in the state(s) you will be employed">
  • A minimum of 2 years of experience in healthcare, preferably in hospice, home health, or clinical liaison roles">
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with healthcare professionals and patients alike">
