Home Health Care Registered Nurse for Weekends

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to join our team as a Home Health Admission Nurse for a nursing job in Harrisonville, New Jersey.

Key Responsibilities:
  • Play a crucial role in setting the stage for successful patient care, safe transitions, and positive outcomes for patients leaving Inspira Hospitals
  • Prevent hospital readmissions, enhance patient satisfaction, and allow more patients to receive their post-acute care in the comfort of their own homes
  • Collaborate closely with transitional care managers, hospital discharge planners, and other healthcare professionals to build strong relationships and ensure seamless care
  • Perform essential duties, including medication administration, wound care, treatments, and client assessments, while closely monitoring patient conditions and documenting findings
  • Work closely with transitional care teams, discharge planners, and population health to support patients as they transition from Inspira Hospitals to home healthcare, ensuring safe and effective care with positive outcomes

Requirements:
  • Ability to work independently, manage time effectively, and communicate well with patients, families, and healthcare partners
  • Solid computer skills, with prior experience in electronic medical records (EMR) preferred
  • Willingness to travel to assigned cases in designated territories
