Job Title: Healthcare Concierge

CVS Health is seeking a highly motivated and compassionate Healthcare Concierge to join our team. As a key member of our Retail Health team, you will play a vital role in delivering exceptional patient care and promoting healthy outcomes.

Job Summary:

This is a dynamic and challenging role that requires strong communication and interpersonal skills. As a Healthcare Concierge, you will work closely with patients, families, and healthcare providers to coordinate care, address health concerns, and provide education and support.

Key Responsibilities:
  • Coordinate patient care and services, including scheduling appointments, managing referrals, and tracking patient progress.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Provide education and support to patients and families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices.
  • Collaborate with healthcare teams to identify and address health concerns, develop care plans, and implement interventions.
  • Maintain accurate and up-to-date patient records, including medical history, treatment plans, and outcomes.
Requirements:
  • High school diploma or equivalent required; associate's degree or higher preferred.
  • Minimum 3-5 years of experience in healthcare, customer service, or a related field.
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
  • Proficiency in Microsoft Office applications, including Outlook, Word, and Excel.
Preferred Qualifications:
  • Certification in medical assisting, pharmacy technology, or a related field.
  • Experience working in a healthcare setting, including hospitals, clinics, or community health organizations.
  • Knowledge of electronic medical records, including Epic.
  • Ability to speak multiple languages, including Spanish.
