Key Responsibilities
  • Collaborate with healthcare professionals to develop and implement individualized patient care plans
  • Conduct thorough assessments and identify patient needs, goals, and barriers to care
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
  • Coordinate patient care team conferences to facilitate goal achievement and outcome monitoring
  • Review patient records and identify potential comorbid conditions, communicating findings to the healthcare team
Requirements
  • Current licensure as a Registered Nurse, Social Worker, or Healthcare Professional
  • Certification in Case Management or Rehabilitation Nursing preferred
  • Minimum 2 years experience in inpatient case management or a related field
  • Excellent communication and interpersonal skills
  • Basic computer skills and proficiency in electronic medical records
