Role Overview: The Healthcare Coordinator plays a vital role in assisting patients with complex health issues to navigate the healthcare landscape. This position focuses on empowering patients through education, resources, and advocacy, ensuring they maintain the highest quality of life. The Healthcare Coordinator collaborates with multidisciplinary teams, including physicians, nurses, pharmacists, and social workers, and serves as a resource for clinic personnel. This role operates in a dynamic, self-directed environment, performing nursing duties within the scope of RN licensure.

Key Responsibilities:

  • Identify high-risk patients in collaboration with providers and clinic staff.
  • Support the ongoing care of patients with chronic conditions by:
    • Conducting health assessments.
    • Performing medication reconciliations.
    • Facilitating motivational interviewing and self-management goal setting.
    • Providing patient education.
    • Creating referrals to appropriate services and resources.
  • Assist in transitioning patients from inpatient to outpatient care by:
    • Assessing transitional needs.
    • Conducting medication reconciliations.
    • Establishing and reviewing contingency plans.
    • Providing patient education.
    • Helping with post-discharge needs such as prescriptions and appointments.
  • Coordinate with healthcare providers to establish or update care plans.
  • Ensure accurate and timely documentation in electronic medical records.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings as required.
  • Maintain competence in nursing practices and stay updated on evidence-based practices.
  • Perform clinical tasks within the scope of practice as needed.
  • Complete other related duties as assigned.

Required Qualifications:

  • Bachelor's degree in Nursing or an Associate's degree in Nursing with 4+ years of RN experience.
  • Active and unrestricted RN license in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a clinical or hospital setting.
  • Knowledge of chronic diseases, including COPD/asthma, diabetes, CHF, and IHD.
  • Proficient in using electronic medical records.
  • Ability to work effectively with individuals and multidisciplinary teams.
  • Excellent verbal and written communication skills.
  • Strong interpersonal, organizational, and prioritization skills.
  • Compliance with health screening requirements.

Preferred Qualifications:

  • Experience in patient education and motivational interviewing.
  • Fluency in English and Spanish, both written and verbal.

Physical & Mental Requirements:

  • Ability to lift and move up to 35 pounds.
  • Ability to sit or stand for extended periods.
  • Proficient in using fine motor skills for office equipment.
  • Ability to comprehend and follow verbal and written instructions.
  • Logical reasoning skills for problem-solving.
