Key Responsibilities
  • Resident Care: Provide direct care to residents, including administering medications, monitoring vital signs, and assisting with daily living activities.
  • Assessment and Documentation: Conduct regular assessments of residents' health and well-being, documenting any changes or concerns in their care plans.
  • Team Collaboration: Work closely with our care team, including RNs, CNAs, and RAs, to ensure seamless care delivery and address any resident needs.
  • Medication Management: Assist with medication administration, monitoring, and documentation, ensuring accurate and timely delivery of medications.
  • Communication: Maintain open and effective communication with residents, families, and healthcare providers to ensure comprehensive care and address any concerns.
Requirements
  • Current LPN License: Possess a current LPN license in the applicable state and CPR certification.
  • Experience: Minimum 1 year of experience in long-term care or a related field.
