Job Summary

The Nurse Practitioner - Geriatric Care Specialist will provide high-quality, patient-centered care to PACE Southeast Michigan participants. This role requires a strong understanding of geriatric care principles and the ability to work collaboratively with the interdisciplinary team.

Key Responsibilities
  • Medical Management: The Nurse Practitioner will assume responsibility for the medical management and planning of participant care, working under the direction of physicians.
  • Collaboration and Communication: The Nurse Practitioner will work closely with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Assessments and Diagnoses: The Nurse Practitioner will conduct periodic assessments, physical examinations, and diagnose common health problems, working in collaboration with physicians.
  • Education and Support: The Nurse Practitioner will provide education to participants, families, and team members, promoting health literacy and self-management skills.
  • Quality Improvement: The Nurse Practitioner will participate in quality improvement initiatives, contributing to the development of nursing policies and procedures.
Requirements
  • Education: Master's degree in Nursing with a clinical nursing focus and nurse practitioner license.
  • Experience: Three to five years of progressively more responsible experience as an RN in a clinical setting, with expertise in geriatrics.
  • Skills: Strong communication and interpersonal skills, with the ability to establish and maintain relationships with participants, families, and team members.
