We are seeking a highly skilled Pharmacist Consultant to join our team at Community Pharmacy, Inc. as a key member of our clinical services team. The ideal candidate will possess a strong background in Long-Term Care Pharmacy and have experience in providing exceptional clinical services to patients in various care settings.
Key Responsibilities- Medication Management Services: Provide comprehensive medication management services, including transitions of care reviews, medication regimen reviews, interim medication reviews, medication reconciliation, drug regimen reviews, and disease state management reviews.
- Medication Regimen Reviews: Conduct medication regimen reviews at the time of request, monthly, or per patient acuity level, and report findings in writing to the facility administrator, director of nursing, and physician.
- Liaison Services: Act as a liaison between facility and pharmacy staff, as well as between the facility, resident, and provider.
- Customer Retention: Engage in customer retention activities with facility staff.
- Emergency Drug Supply Kits: Assist in the development of emergency drug supply kits in conjunction with the medical director, provider pharmacy, and nursing administration.
- Quality Assurance: Attend quality assurance and other committee meetings as requested.
- Quarterly Quality Assurance Reports: Prepare quarterly quality assurance reports.
- Education and Counseling: Provide education to residents preparing for discharge, either directly or indirectly, and offer medication counseling in person or via teleconference to residents in care facilities or at home.
- In-Service Education: Plan and present in-service education to pharmacy staff and nursing facility employees as needed.
- Pharmacy Policies and Procedures: Stay informed of and implement relevant pharmacy policies and procedures.
- CMS Regulations and State Laws: Maintain knowledge of CMS regulations, interpretive guidelines, and applicable state and federal pharmacy laws.
- Continuing Education: Pursue continuing education relevant to the practice setting.
- Field Technician Supervision: Supervise field technicians as appropriate.
- Education: B.S. Pharm or Pharm D. degree from an accredited School of Pharmacy.
- License: Current pharmacy license in the state(s) of practice.
- Experience: Experience in Long-Term Care Pharmacy or Consulting in Long-Term Care.
- Driver's License: Valid driver's license without restrictions.
- Skills: Professional demeanor, strong organizational and clinical pharmacy skills, excellent oral and written communication abilities, proficiency in learning new technology applications, flexible 7-day availability, willingness to travel within the territory, and ability to work independently and as part of an interdisciplinary team.
