Job Summary

We are seeking a highly skilled and dedicated professional to join our team as a Health Program Coordinator. This is an exciting opportunity to work in a dynamic environment and make a meaningful contribution to the health and well-being of our community.

Key Responsibilities
  • Develop and implement community health programs and activities that promote healthy behaviors and prevent disease.
  • Collaborate with community partners and stakeholders to identify health needs and develop strategies to address them.
  • Provide education and training to community members on health-related topics and promote healthy behaviors.
  • Conduct outreach and engagement activities to promote health programs and services.
  • Monitor and evaluate program effectiveness and make recommendations for improvement.
Requirements
  • Bachelor's degree in public health, social work, or a closely related field.
  • Four years of professional experience in a direct care setting coordinating mental health or substance abuse services, or four years of experience with evidence-based programs in violence prevention and behavioral health.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a strong commitment to customer service.
