Community Care Coordinator

We are seeking a compassionate and detail-oriented individual to join our team as a Direct Support Services Provider. This role involves working closely with consumers to develop and implement personalized plans that promote independence and quality of life.
**Responsibilities:*
- Conduct assessments to identify consumer needs and develop individualized service plans.
- Assist consumers with daily activities, such as meal preparation, cleaning, and personal care.
- Monitor and report any changes in consumer behavior or condition.
**Requirements:*
- High school diploma or equivalent required; Bachelor's degree preferred.
- 1-2 years of experience working with individuals with disabilities.
- CPRP/RPRP certification desired.
