Job Summary:The RN - Care Transition Nurse Ambulatory - Family Medicine Hoover will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare teams to ensure seamless care transitions. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Assess patient needs and develop individualized care plans
- Coordinate with healthcare teams to ensure seamless care transitions
- Provide education and support to patients and their families
- Collaborate with interdisciplinary teams to achieve patient-centered care
- Document patient information and care plans accurately and efficiently
- Current Alabama RN license
- Minimum of two years clinical experience
- BSN preferred
- Current CPR certification or must be obtained within 30 days of employment
