Job Summary

We are seeking a highly skilled Medication Reconciliation Technician to join our pharmacy team at Penn State Health Lancaster Medical Center. As an integral member of our team, you will work closely with newly admitted patients in the emergency department to obtain and document an accurate medication list, ensuring the prevention of medication duplications, omissions, and errors.

Key Responsibilities
  • Obtain and document accurate medication lists for newly admitted patients
  • Enter medication information into the electronic health record
  • Collaborate with physicians and other healthcare providers to determine medication orders
  • Provide excellent customer service and communication skills
  • Work effectively with patients, staff, and outside providers
Requirements
  • High School Diploma or equivalent required
  • Current Pharmacy Technician certification or licensed practical nurse by state of employment
  • Minimum three years of medication history or pharmacy experience preferred
