Primary Care Nurse Practitioner for Senior Patients

We are seeking a compassionate and skilled Primary Care Nurse Practitioner to join our team at Abode Care Partners.

**Responsibilities:**
Conduct evidence-based medical assessments and develop comprehensive care plans for patients in various settings, including home, assisted living facilities, and independent living communities.
Manage chronic illnesses, minor injuries, and urgent conditions, ensuring timely and effective interventions.
Collaborate with healthcare professionals, caregivers, and other stakeholders to optimize patient outcomes and ensure seamless care coordination.
Provide referrals to other healthcare practitioners as needed and maintain accurate and up-to-date patient medical records.
**Required Skills and Qualifications:**
A Master's degree in Nursing (MSN) and active state Nurse Practitioners license are essential.
Current Nurse Practitioner National Board Certification and active state DEA licensure are also required.
Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
A strong knowledge base of HIPAA and OSHA regulations, proficiency in electronic medical record systems, and excellent communication skills are necessary.
