Position Summary
Our Pharmacy division is dedicated to delivering outstanding customer service and addressing the pharmaceutical needs of healthcare facilities, including hospitals, rehabilitation centers, long-term acute care facilities, and specialized care centers across the nation.
Key Responsibilities
- Assess physician medication orders for suitability regarding drug choice, dosage, potential interactions, and administration routes. Provide recommendations for therapy modifications to physicians and facility personnel as necessary.
- Maintain a comprehensive patient/resident database that includes relevant current and historical medical information, clinical and laboratory data, and ongoing medication records (including over-the-counter and home remedies).
- Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, reviewed, and communicated with appropriate healthcare team members.
- Collaborate with the healthcare team to deliver clinical and technical support as required.
- Document all interactions with physicians, nurses, other healthcare team members, and patients/residents, along with their families, in the medical record.
- Implement policies and procedures for the safe acquisition, storage, distribution, and utilization of medications and biological products, as well as their disposal.
- Obtain and maintain all required documentation in compliance with federal and state regulations and organizational policies.
- Notify the Pharmacy Manager of any medication-related errors.
- Assist facility staff with procedures related to the safe and effective management of medications.
- Perform additional tasks as assigned.
Qualifications
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- Minimum of one year of experience in long-term care settings.
- Licenses/Certifications: As mandated by regulatory requirements and business needs.
- Detail-oriented, goal-focused, team-oriented, with strong oral communication skills.
