Job Summary

We are seeking an experienced Support Nurse - LPN to join our team at Liberty Healthcare and Rehabilitation Services. As a Support Nurse, you will play a vital role in providing high-quality patient care and supporting our nursing staff.

Key Responsibilities
  • Assist in Providing Resident Care: Assist nursing personnel, including charge nurses and Directors of Nursing, in providing resident care on a specified unit during assigned work hours.
  • Take Call for Staffing Coverage: May be required to take call for staffing coverage to ensure adequate staffing levels.
  • Delegate Duties to Non-Licensed Nursing Personnel: Delegate duties to non-licensed nursing personnel and monitor their performance under the supervision of a Registered Nurse.
  • Assist with Quality Assurance Data Collection: Assist in collecting and analyzing quality assurance data to identify areas for improvement.
  • Support Infection Control Efforts: Assist with infection control efforts on the unit, adhering to all regulations and guidelines.
  • Assist with Records and Orders: Assist with maintaining accurate records and orders, including initial nursing assessments and resident assessments.
  • Attend Nurse Department Meetings: Attend nurse department meetings as scheduled to stay informed and provide input.
  • Monitor and Measure Decubiti: Monitor and measure decubiti weekly and complete integument watch assessment forms accurately.
  • Report Performance Issues: Report any possible performance issues by employees within the facility to the RN for determination of disciplinary action.
  • Assist with Lab Requests: Assist with lab requests as ordered, using appropriate techniques and protocols.
  • Respond to Emergencies: Respond to emergencies arising while on duty, including performing CPR if necessary.
  • Notify Attending MD of Changes or Illnesses: Notify attending physicians of any changes or illnesses of patients on the unit.
  • Implement Orders and Changes: Implement orders and changes as instructed or ordered by the MD.
  • Coordinate Health Care Team: Assist with coordinating the health care team, including rehab aides and other staff members.
Requirements
  • High School Diploma or Equivalent: Must be a high school graduate or equivalent.
  • LPN License: Must be a Licensed Practical Nurse (LPN) graduated from an accredited School of Nursing, with a current, valid license as a Licensed Practical Nurse in North Carolina.
  • CPR Certification: Must be CPR certified yearly and attend in-service training and other staff meetings as required.
  • Ability to Make Decisions: Must have the ability to make decisions regarding nursing and unit problems, realizing that errors and incompetence may have serious consequences for patients and/or staff.
  • Ability to Work Well Under Pressure: Must be able to work well under pressure, problem solve, and perform various jobs.
  • Knowledge of Policies and Procedures: Must read, know, and follow personnel, department, and facility policies and procedures, and adhere to local, state, and federal requirements.
