Salary: The estimated salary for this role is $65,000 - $85,000 per year, depending on experience.

Job Description:

  • Complete resident assessments and develop care plans that meet current regulations and requirements.
  • Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and comprehensive assessments of physical, mental, and psychosocial status.
  • Maintain open communication with healthcare teams to ensure seamless transitions and high-quality care.

Required Skills and Qualifications:

  • Completion of an approved nursing school program.
  • Experience with MDS, resident assessment, care planning, and CAAs preferred.
  • Current licensure as a registered nurse in the state.
  • Excellent communication and teamwork skills, with a patient and tactful demeanor.
