Key Responsibilities- Collaborate with Nurse Practitioners and Physician Assistants to manage resident care
- Provide comprehensive primary care services, including diagnosing and treating acute and chronic conditions
- Document patient information in Theorias proprietary electronic health record system, ChartEasyTM
- Participate in medical quality initiatives and programs, including Chronic Care Management and Transitional Care
- Ability to supervise collaborating nurse practitioners and physicians assistants
- Proficiency in electronic/online medical tools, including EHR systems and state/county medical systems
- Flexibility to work a minimum of 1-2 days/week at each facility, with flexible rounding times
