Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Job Summary:

The Home Health Occupational Therapist will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment, the Occupational Therapist will develop a treatment plan, perform interventions, and evaluate patient progress.

Responsibilities:
  • Evaluate patient functional status and occupational therapy needs, consulting with physicians as needed.
  • Develop and implement treatment plans, using evidence-based techniques and coordinating with the healthcare team.
  • Provide education and training to patients, families, and caregivers on functional limitations and progress towards goals.
  • Complete progress notes and submit them according to LHC policy.
  • Report outcomes and progress to nurse supervisors within 24 hours.
  • Participate in staff conferences and committees as requested.
  • May supervise home health aides and occupational therapy assistants, as per LHC policy and state regulation.
Requirements:
  • Current Occupational Therapy licensure in the state of employment.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to adapt to changing situations and priorities.
