Job Summary

The successful candidate will be responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This will involve assessing patients for transition needs, identifying and assessing patients at risk for readmission, and conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.

Key Responsibilities
  • Assess patients for transition needs and develop effective care plans
  • Conduct complex psycho-social assessments and interventions to promote timely throughput and safe discharge
  • Identify and assess patients at risk for readmission and develop strategies to prevent readmissions
  • Collaborate with physicians, office staff, and ancillary departments to ensure seamless care coordination
  • Communicate effectively with patients and families about the plan of care
  • Document patient information accurately and timely in the Case Management system
Requirements
  • Master's of Social Work degree
  • Current licensure, certification, or registration to practice as a Social Worker
  • 2 years of acute hospital experience preferred
