Job Summary:

We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our team at Willows Post Acute. As a Licensed Vocational Nurse, you will play a vital role in providing high-quality care to our residents.

**Responsibilities:**

* Provide direct care to residents, including administering medications, conducting daily rounds, and monitoring resident care provided by unlicensed staff.
* Maintain the confidentiality of all resident care information and promote a culture of safety to ensure a healthy practice and living environment.
* Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
* Participate in ongoing education and training to stay current with best practices and regulatory requirements.

**Requirements:**

* Current licensure to practice as a Licensed Vocational Nurse in the state.
* Strong communication and organizational skills.
* Basic knowledge of computer use or willingness to learn.

**Nice to Have:**

* Experience in Skilled Nursing Facility (SNF) environment.
* Nursing experience providing direct care to multiple residents.
* Behavioral care / memory care experience.
* Experience with Point Click Care (PCC).
