Pyramid Healthcare is in search of a Substance Abuse Aftercare Advisor to enhance our dedicated team.

Work Schedule: 40 hours per week

Compensation: $42,000 - $45,000 annually

Position Overview:
This role is pivotal in ensuring that clients transition from treatment with a comprehensive aftercare strategy, secure housing, and any additional necessary support. The Aftercare Advisor will play a crucial role in achieving our overall operational and financial objectives.

Key Responsibilities:

  • Deliver quality direct and indirect care to individuals facing substance use challenges.
  • Evaluate clients' ongoing care and referral requirements.
  • Build strong relationships with clients to foster trust.
  • Gather essential information and permissions from clients, families, and other relevant parties.
  • Facilitate referrals to mental health and substance use treatment facilities.
  • Coordinate interviews and placements at halfway houses.
  • Collaborate closely with counselors and the multidisciplinary team.
  • Participate in program, clinical, and administrative meetings.
  • Support the facility's referral initiatives actively.
  • Engage in mandatory training and orientation sessions.
  • Perform additional duties as assigned.

Qualifications:
Education and Experience:
A high school diploma is required; a Bachelor's degree in a related human services field is preferred.

Knowledge, Skills, and Abilities:
To excel in this position, candidates must demonstrate the ability to perform each essential duty effectively. The following skills are essential:

  • Strong oral and written communication skills.
  • Understanding of clinical care levels, funding sources, and referral networks.
  • Problem-solving capabilities through data gathering and analysis.
  • Ability to manage crisis situations effectively.
  • Familiarity with federal and state regulations and standards.

Physical Requirements:
The role requires frequent communication, regular sitting, and the use of hands for various tasks. Occasional lifting of up to 25 pounds is necessary, with rare instances of lifting up to 50 pounds. Vision requirements include close, distance, and peripheral vision.

Work Environment:
The work setting is typically quiet and indoors. Scheduling will depend on operational needs, and personal transportation is required. The position may involve exposure to health risks from clients and potentially traumatic situations.
