Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse to join our team at Equiliem Healthcare. As a LPN, you will be responsible for providing high-quality patient care to individuals with developmental disabilities in our community-based settings.
Job Summary:The Licensed Practical Nurse will work under the supervision of a Registered Nurse to provide direct patient care, administer medications, and perform various medical procedures. The ideal candidate will have excellent communication skills, a strong commitment to patient-centered care, and the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide direct patient care, including administering medications, performing medical procedures, and documenting patient information.
- Work collaboratively with the healthcare team to develop and implement individualized care plans.
- Conduct regular assessments to identify patient needs and develop strategies to address them.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current LPN licensure in New York State.
- Minimum 1 year of experience in a healthcare setting, preferably with individuals with developmental disabilities.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
