Nurse Case Manager

About the Role:We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for conducting in-home nursing visits for our clients, who are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government.
Responsibilities:- Conduct weekly in-home nursing visits, typically lasting 1-2 hours per visit
- Perform physical assessments, including vitals, and ensure home safety with medical equipment and supplies
- Fill weekly medication planners and oversee skilled and non-skilled caregivers providing care in the client's home
- Provide case management to assigned clients, directing a team of RNs, LPNs, and caregivers in the delivery of care
- Participate in client care conferences as needed and serve as a local on-site clinical resource
- Assist and collaborate with staffing coordinators regarding staffing and scheduling of personnel
- Adhere to nursing delegation guidelines and ensure adherence to Agency policies
- Graduate of a state-approved school of professional registered nursing
- Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field
- Provide direct care and case management of assigned clients
- Plan, implement, and evaluate care provided
- Competitive hourly rate of $00 plus shift differentials, where applicable
