About the Role:

We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for conducting in-home nursing visits for our clients, providing physical assessments, ensuring home safety, and overseeing skilled and non-skilled caregivers. You will also be responsible for case management, including planning, implementing, and evaluating care provided to our clients.

Responsibilities:
  • Conduct in-home nursing visits for clients, including physical assessments and home safety evaluations
  • Provide case management services, including planning, implementing, and evaluating care provided to clients
  • Oversee skilled and non-skilled caregivers providing care in clients' homes
  • Participate in client care conferences as needed
  • Collaborate with staffing coordinators to ensure appropriateness of staffing and scheduling of personnel
  • Adhere to nursing delegation guidelines and Agency policies
  • Participate in performance improvement initiatives, including quarterly medical record review
  • Collect, document, and submit data on infections, occurrences, complaints, and grievances
Requirements:
  • Graduate of a state-approved school of professional registered nursing
  • Minimum of two years of nursing experience, including one year in home care or a closely related field
  • Ability to provide direct care and case management to assigned clients
  • Ability to plan, implement, and evaluate care provided
  • Ability to participate in client care conferences and collaborate with staffing coordinators
