Responsibilities

Position Summary:

Under the guidance of the Supervisor Care Manager, the incumbent will assess and evaluate potential clients holistically, identifying functional limitations that hinder independent living.

Qualifications

Minimum Qualifications:

A minimum of two years of experience in social services, particularly in aging, health, and long-term care sectors is required. A Bachelor’s degree in social work from an accredited institution is mandatory (BSW). A valid CA Driver's license is essential. BLS Certification from an American Heart Association program is required. Strong interpersonal skills to connect with clients and their families. Exceptional communication and presentation abilities are necessary.
