We are seeking a highly skilled and compassionate Patient Portal Representative, LVN to join our team at Family Health Centers of San Diego. As a key member of our healthcare team, you will be responsible for facilitating and resolving electronic patient portal requests to meet the needs of our patients.
Responsibilities- Respond to patient requests from the patient portal to provide resolution within established guidelines and timeframes.
- Assess patient needs to determine the best course of action to ensure positive patient outcomes, which may include collaboration with internal and external patient care teams.
- Utilize clear and concise electronic written and verbal communication skills to draft correspondence, complete documentation, and respond to others.
- Use company and departmental protocols and procedures to resolve patients' needs.
- Coordinate services with other departments for follow-up.
- Use problem-solving skills to determine the best course of action that will be satisfactory to the patient and organization.
- Maintain up-to-date knowledge and skills in professional, clinical, and organizational technology areas.
- Use critical thinking and communication skills to assess and process patient portal requests based on evidence-based guidelines.
- Accurately document work consistent with internal policies and procedures.
- Graduation from a recognized practical nurse program or equivalent.
- High School Graduate (or GED equivalent).
- Minimum of two (2) years of experience in acute care nursing or outpatient clinic.
- Current Basic Life Support (BLS) and Cardiopulmonary resuscitation (CPR).
- California State Board LVN License required.
- Bilingual English/Spanish is required.
- Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
- Licensed for a minimum of 3 years.
- No more than 2 violations and/or accidents within 3 years.
- No vehicle-related suspensions/reinstatements.
- No DUI, reckless, or felony driving within 7 years.
