Clinical Liaison

Clinical Liaisons at Select Specialty Hospital serve as patient advocates, working closely with healthcare professionals to identify patients who require extended hospital care. This role involves evaluating patients' medical needs and developing relationships with hospitals and healthcare facilities to ensure seamless transitions of care.
Responsibilities
- Perform clinical assessments of referred patients and counsel with patients' families to assist in making informed care decisions.
- Partner with hospital case managers to create smooth patient transfer processes and remove barriers to care.
- Work with physicians, discharge planners, and patients' families to plan the best continuum of care.
Requirements
- Registered Nurse (RN) or Respiratory Therapist (RT) License.
- Strong clinical background, payer knowledge, and problem-solving skills.
- Ability to work autonomously and as part of a team.
