Nurse Specialist

Job Summary:

As a skilled nurse at UNC Health Care, you will play a vital role in providing high-quality patient care in a skilled nursing facility setting. Your primary responsibility will be to assess and document patient status, formulate and implement individualized care plans, and collaborate with other healthcare team members to ensure seamless care delivery.

Key Responsibilities:

• Assess patient's condition, including biophysical, psychosocial, and age-specific needs.
• Develop and implement individualized care plans, utilizing nursing and medical diagnoses.
• Collaborate with other healthcare team members to ensure comprehensive care delivery.
• Evaluate patient's status, nursing instructions, and outcomes of care provided.
• Initiate patient education plans, as prescribed by physicians and/or policy.

Requirements:

• Bachelor's degree (BSN) from an accredited School of Nursing preferred.
• NC RN Licensure required.
• Prefer one year of experience in skilled nursing facility.
